Description
If you have a member on your buddy list and add him to the ignore list, he'll be on both lists and the other way round. Well now which is the dominant one (e.g. posts, pms, etc.)?
I think if you add a user to a list (buddy or ignore) and the user is already on the opposite list, then the user should be removed from the other list by MyBB itself as it is contradictional otherwhise.

I think showing a warning message is the better and simpler way. We just need to check if the user is in buddy/ignore list.

Well at least blocking from being able to add a user on both lists at the same time.
